In another incident, another female resident, who had dementia and was in a wheelchair, wandered into the doorway of the same male resident. The nursing home initially said the allegation was unsubstantiated but after discussing the case with the CMS inspector changed it to substantiated. The report says the nursing home's director of nursing told the laundry supervisor "not to discuss the incident with anyone and it would be taken care of." The laundry supervisor said she had never been trained on what to do in such a situation. A nursing assistant, who later reported that she had seen the male patient repeatedly touched his genitals, immediately removed him from the room. The supervisor ran into the hall and called for help. One morning, the home's laundry supervisor came into the bedroom of a female resident with severe cognitive impairment to find a male resident at her bedside with his hands in her underwear. The citations landed the facility on the candidates list as a result of a CMS inspection report in December, "despite the fact that Canton Christian Home does not have a record of poor survey inspection performance," and "has cared for nearly 400 nursing home residents over the last three years and has had no harm level deficiencies or similar resident behavior issues."
